IN-YEAR APPLICATION FORM (PRIMARY)

Please complete this form to apply for an in-year place at The Piggott School: Charvil Primary.

This form should only be used to apply for a place outside of the normal admissions round (i.e. for places in year groups other than Reception in September).

Please complete one form for each child that you are applying for.

Applications can only be made for the current term (if moving into the area) or for the next half term.

All questions marked with a * must be completed and other sections should be completed where applicable.

Applications can take up to 15 school days to be processed. Please be aware that applications submitted in school holidays may take longer to be processed.

• a utility bill (not mobile phone bill) dated in the last 3 months showing the new address
• a credit card or bank statement in the last 3 months showing the new address
• any letter or official document from central/local government such as Department for Work and Pensions, the Employment Service, Social Security showing the new address
• a benefit statement dated in the last 3 months showing the new address
• a visa UK or work permit showing the new address
• a letter from the local doctors stating that the student is registered with them showing the new address
